Safety Committee

POLICY:
 
The purpose of the Safety Committee is to assure a safe 
environment for patients, visitors, employees and 
professional staff members.  Meetings are held on a 
regularly-scheduled basis.  The Safety Committee addresses 
requirements related to the staffing, equipping, operation, 
and maintenance of the facility with the goal of producing 
safe characteristics and practices, and eliminating/reducing 
hazards.
 
One means of meeting the above goal is the development and 
promotion of safety training and educational activities for 
all facility personnel.  The Committee uses various modes of 
communication for the distribution and dissemination of 
accepted safety standards and practices, including in- 
service and continuing education, motivational posters, a 
safety bulletin board, and the Safety Manual.  The Committee 
is also responsible for conducting a monthly safety 
inspection.
 
COMPOSITION OF COMMITTEE
 
The Safety Committee is composed of several staff members, 
appointed by the Medical Director, or, as appropriate, of all 
members of the ancillary staff.
 
The Safety Officer is designated by the Medical Director and 
is the Committee Chair.
 
FORMAT AND STANDING AGENDA
 
Minutes and a list of attendees will be kept at each meeting.

Agenda: Review of monthly safety inspections 
        Review of incident reports
 
New business: Newly-recognized hazards
 
Old business: Action taken since last meeting; work in 
              progress
 
Review of disaster protocols
 
RESPONSIBILITIES OF SAFETY COORDINATOR
 
     -Provide for a monthly safety inspection of all areas of 
      the organization.
 
     -Adopt or establish an inspection form that is complete 
      and pertinent to the equipment and environment.
 
     -Establish and maintain a file that includes a record of 
      inspection, type of hazards found, action taken and 
      date of correction.
 
     -Notify the supervisor of any employee/professional 
      staff member who is consistently unavailable for 
      training sessions; missing any three training sessions 
      during a 12-month period (applicable to the employee's 
      position) may be grounds for disciplinary action, up to 
      and including termination.
